Hi everyone, Kevin here. Today I want to show you how you can create your very own fillable form using Microsoft Word. First off, what is a fillable form? You can have people go in and fill out a form, but they wonamp;#39;t be able to edit the questions or the structure of the form. You can use things like check boxes, text fields, drop down lists, date pickers, to build your form. Once someoneamp;#39;s finished completing your form, they can e-mail it back, they could print it out, or you could even connect it to a database. Connecting to a database is outside of the scope of todayamp;#39;s tutorial. Itamp;#39;s kind of like an docHub PDF form, but itamp;#39;s Microsoftamp;#39;s version of it. Iamp;#39;ve included sample files today if you want to follow along, otherwise letamp;#39;s jump on the PC and letamp;#39;s get started. Take the difference between the current day and their birth day: days = current day - birth day . Replace these differences in the age formula: age = (years 365) + (months 31) + days . This is the persons age in days. Divide the result by 365 to get the age in years.
Open Word and make the developer tabvisible on the ribbon, which displays the settings and tools at the top of the screen. You can now create fillable lines. Intake forms can be made in Word by following these steps: Step 1 : Open Microsoft Word. Step 2 : Make a new document. Step 3: Add form fields. Step 4: Customize the details of the form. Step 5: Keep the form safe. Step 6: Send it out and save it.
Insert a date that updates automatically On the Insert tab, in the Text group, click Date Time. In the Date and time dialog box, select the format you want. Select the Update automatically check box. The date is inserted as a field and will update automatically.
Writing ages You can also refer to ages as [age]-year-old. Always include the hyphens. For this analysis, we looked at 15- to 45-year-old women. You can include the months or weeks for ages under one year if needed.
In Word, you can create a form that others can fill out and save or print.
