There are different types of dysplasia. Mild dysplasia, called low-grade intraepithelial lesion (LSIL) is one type. Moderate or severe dysplasia, called high-grade intraepithelial lesion (HSIL) is another type of dysplasia. LSIL and HSIL may or may not become cancer.
It is more likely than LSIL to be linked to precancer and cancer. HSIL stands for high-grade squamous intraepithelial lesion. ASC-HASC-H means that changes in the cervical cells have been found that raise concern for the presence of HSIL. ASC-H stands for atypical squamous cells, cannot exclude HSIL.
LSIL (also called low-grade squamous intraepithelial lesion, or mild dysplasia) seen on a Pap test is generally CIN 1. HSIL (also called high-grade squamous intraepithelial lesion, or moderate or severe dysplasia) seen on a Pap test can be CIN 2, CIN2/3, or CIN 3.
Untreated LSIL can progress in 13% of cases to HSIL during 2 years of monitoring (9,10). Women infected with HPV 16 have a transition of LSIL into HSIL in 9% of cases.
LSIL is now recommended to be used as a diagnostic category to describe HPV transient infection-related changes, while HSIL is used to categorize true precancerous lesion.
High grade squamous intraepithelial lesion (HSIL) changes consist of immature, delicate or metaplastic cytoplasm. The nuclear features generally contain high N/C ratios, nuclear envelope irregularities and generally hyperchromatic nuclei, although hypochromatic forms can be present.
