Syncope could be the initial complaint in 5-12% of patients with acute myocardial infarction (MI) and is related to arrhythmia or pump failure. Vasovagal reactions, bradyarrythmia and atrio-ventricular blocks can be caused by ischemia.
In primary pulmonary hypertension, and in any condition that leads to acute changes in right ventricular wall stress, syncope can occur due to several autonomic mechanisms, including a vasovagal reflex due to activation of ventricular mechanoreceptors.
Loss of consciousness can occur in myocardial infarctions due to inadequate blood flow to the brain and cardiogenic shock, and sudden death, frequently due to the development of ventricular fibrillation.
Symptoms Chest pain that may feel like pressure, tightness, pain, squeezing or aching. Pain or discomfort that spreads to the shoulder, arm, back, neck, jaw, teeth or sometimes the upper belly. Cold sweat. Fatigue. Heartburn or indigestion. Lightheadedness or sudden dizziness. Nausea. Shortness of breath.
Syncope is almost twice as common in people with cardiovascular disease than those without. In acute ischaemia, the mechanism can be vasovagal (especially in inferior MI), high grade AV block or tachyarrhythmias.
Myocardial infarction (MI), colloquially known as heart attack, is caused by decreased or complete cessation of blood flow to a portion of the myocardium. Myocardial infarction may besilent, and go undetected, or it could be a catastrophic event leading to hemodynamic deterioration and sudden death.
Becoming unconscious due to a seizure, heart attack, head injury, stroke, intoxication, blow to the head, diabetic hypoglycemia or other emergency condition is not considered syncope.
Alternatively, there may be interference as the descending autonomic pathways course through the brainstem and spinal cord [6]. ANS dysfunction explains the orthostatic symptoms including syncope, near syncope, and orthostatic hypotension that have previously been reported in MS patients [7, 8].
