This procedure involves using a specialized instrument known as a colposcope to examine the cervix, vagina, and vulva closely. By providing a magnified view of these areas, colposcopy enables healthcare professionals to identify and evaluate abnormalities that may not be visible to the naked eye.
Abnormal result This means the tests done during colposcopy or biopsy have found there are cell changes in your cervix. Abnormal results may tell you that you have the following: cervical intraepithelial neoplasia (CIN) cervical glandular intraepithelial neoplasia (CGIN)
Atypical patterns. Abnormal vessel patterns that do not appear mosaic or punctate include corkscrew vessels, comma-like vessels, and character-writing-type vessels. These are associated with high-grade lesions or microinvasive and invasive disease.
The atypical vessel patterns are varied and may take the form of hairpins, corkscrews, waste thread, commas, tadpole and other bizarre, irregular branching patterns with irregular calibre. Most glandular lesions originate in the transformation zone and may be associated with concomitant CIN lesions.
HPV positive women with suspected cancer on VIA are to be appropriately referred for further assessment and biopsy. The VIA positive women are assessed for suitability of ablative treatment and treated with either cryotherapy or thermal ablation at the same sitting.
fig 2.19: VIA positive: There is well-defined, dull, dense, opaque acetowhite area, with raised and rolled-out margins in the anterior lip abutting the squamocolumnar junction which is fully visible. The lesion is extending into the cervical canal.
Normal mature squamous epithelium contains glycogen. When in contact with Lugols iodine it becomes black, whereas precancerous lesions and cancer contain little or no glycogen thus turning yellow after Lugol application. Such a reaction is considered to be VILI positive.
Normal colposcopic findings Identify the columnar epithelium, and upper limit of the TZ. Look for and note the following normal findings: ectopy, metaplastic squamous epithelium (mature or immature), nabothian cysts, crypt (gland) openings, deciduosis in pregnancy or atrophy.
