You can enroll in our Assurance Wireless Unlimited plan if you participate in one or more government assistance programs, such as SNAP or Medicaid. You may also qualify based on your household income. Please be prepared to provide proof of program participation or proof of income during the application process.
There are three ways to redocHub for Lifeline. Complete the online form to redocHub. This option is only available if you DO NOT have to provide proof documentation. Call (855) 359-4299, enter in the application ID number included in your letter, and follow the prompts.
If your income increases or you no longer participate in the government assistance program that you signed up with, you will lose the Lifeline benefit (or your service will be turned off). Lifeline rules require that you use your service every 30 days.
An unlocked device is no longer locked to Assurance Wireless services and the user may attempt to activate it on another carriers network. How can I unlock my device? Your phone can be unlocked when you are eligible for unlock.

Assurance Wireless is only available to individuals who participate in a qualifying government program or are income eligible. Only one wireless or wireline Lifeline account is allowed per household and service is non-transferable.
If you do not place a call, send a text or go online using your plan data within 15 days of receiving a termination notice, your service will be terminated. You will no longer be able to use your FREE Assurance Wireless phone service and will need to re-apply.
