There can be a high rate of false positives or missed diagnoses. Additionally, ACL tears can occur with other injuries that should not be missed.
Magnetic resonance imaging (MRI) is commonly used for diagnosis and as a research tool, but its accuracy is questionable. After the MRI only 1 out of 6 received appropriate treatment. MRI confirms what you already told your doctor, you have knee pain. Research says 43% of Knee MRIs are arguably useless.
Secondary signs include bone marrow edema (secondary to bone contusion) in more than half of ACL tears. The characteristic ACL bone bruising involves the middle third of the Lateral femoral condyle and the posterior third of the lateral tibial plateau.
The Lachman test is a specific clinical exam technique used to evaluate patients with a suspected anterior cruciate ligament (ACL) injury. The test relies on proper positioning and technique and is regarded as the most sensitive and specific test for diagnosing acute ACL injuries.
Symptoms of ACL Injuries Popping sound or sensation in the knee. Swelling and inflammation, usually occurring quickly after injury. Pain severity can vary depending on the extent of the injury. Reduced range of motion in the knee. Instability when placing weight on the affected leg. Inability to walk or continue activity.
Partial Anterior Cruciate Ligament Tears Partial tears are often overlooked on MRI and are difficult to evaluate clinically and on routine arthroscopy, particularly when the ACL synovial envelope is intact [57, 58].
It can be easy to confuse the two injuries, as both a torn ACL and a torn MCL exhibits similar symptoms, including: swelling, inflammation, extreme pain and possible bruising. The main difference between an ACL tear and an MCL tear is that an ACL tear will have a distinctive popping sound, while an MCL tear will not.
Its not unusual for patients who are suffering with knee pain to have a negative MRI. This situation is relatively common. In cases of a strain or inflammation the MRI may indeed be negative for structural problems.
