No initials should be written and all initials (if any) in the applicants name should be expanded. For instance, for the name used above, writing the Given Name as P.K.JAIN or PIYUSH K JAIN is not correct. No honorifics, titles such as Major, Doctor etc should be written.
When writing a name initials are always capitalized and followed by a period. Note that when writing both initials you do not include a space between, so J.S. , NOT J. S.
Fill all details in the form in capitals, with one letter in each box. Avoid using fancy fonts ,and use standard fonts only. Use a blue or black ballpoint pen and not a pencil or ink pen. Mark a cross in the boxes where you have to choose one or more options as your answer and leave the other option(s) blank.
Your surname is your family name. Its also called your last name. When filling out applications, type your surname as it appears on your passport, travel or identity document. Do not use initials.
The passport is a federally issued identification document so be sure to use your full legal name. Middle names can be tricky when filling out your passport application, but dont let that be the one factor that holds you back. A middle initial is acceptable on your passport instead of providing the full middle name.
If the ID you present with the application supports the name you enter in the form and other details match (birth date, birthplace), the passport should be issued in the name you currently use. Answer: The suffix (or lack thereof) will not cause any issues with your application.
Sometimes an applicant will receive their passport back from the U.S. Passport Agency and their name will be misspelled or the gender is issued incorrectly. Any error on your passport MUST be corrected otherwise your passport may not be considered valid to use for travel.
