Generally you can choose how you want your name displayed on a certificate. However, it is customary to have at least your first name and family name spelled in full. Any other names can be substituted with just the first letter.
How do you write name on a certificate? The names on most certificates appear as full first name, middle initial, and full last name. If a recipient goes by their middle name instead of their first name, you can spell out the middle name and keep the first name as an initial.
Should you include your middle name on certificate? You should use whichever name you would prefer to see on the diploma when you look at it. If you like your middle name, then by all means include your full middle name. If you dont like your middle name, then use your middle initial.
Your name on your diploma will be printed as your full legal name as it appears on your student record.
Your surname is your family name. Its also called your last name. When filling out applications, type your surname as it appears on your passport, travel or identity document. Do not use initials.
The correct format should be - First of all, your name should come in the beginning and then your surname. For example- SUBHAM CHAKRABORTY. Here, my name is written in bold and the surname is written in italics. If you are not married,then Misswill be added before your name if married then Mrs. will be added.
No initials should be written and all initials (if any) in the applicants name should be expanded. For instance, for the name used above, writing the Given Name as P.K.JAIN or PIYUSH K JAIN is not correct. No honorifics, titles such as Major, Doctor etc should be written.
How to Write a Name on an Award, Certificate, Diploma or Plaque #1) It can be their formal name if its a formal award, a nick-name or go-by name for an informal award. #2) Include nothing before their name: no rank, no honorific, no courtesy title.
