Five Wishes lets your family and doctors know: Who you want to make health care decisions for you when you cant make them. The kind of medical treatment you want or dont want. How comfortable you want to be. How you want people to treat you. What you want your loved ones to know.
Is the Five Wishes advance directive a legal document? Yes. It was written with the help of the American Bar Associations Commission on Law Aging. It meets the legal requirements of 46 states, but is used widely in all 50, and a federal law requires medical care providers to honor patient wishes as expressed.
Five Wishes is the first living will (also called an advance directive) that talks about your personal, emotional, and spiritual needs as well as your medical wishes. It lets you choose the person you want to make health care decisions for you if you are not able to make them for yourself.
Think of it as a living will or as a conversation piece you can use to collect all your end-of-life wishes in a single place. Five Wishes is a legal document in all states but eight. Alabama, Indiana, Kansas, New Hampshire, Ohio, Oregon, Texas and Utah all require their own official documentation.
You can put any wishes you have for medical care in your living will. What to Put in Your Living Will Life-prolonging medical care. These treatments include: blood transfusions, CPR, diagnostic tests, dialyses, administration of drugs, use of a respirator, and surgery. Food and water. Palliative care.
The Five Wishes Wish 1: The Person I Want to Make Care Decisions for Me When I Cant. Wish 2: The Kind of Medical Treatment I Want or Dont Want. Wish 3: How Comfortable I Want to Be. Wish 4: How I Want People to Treat Me. Wish 5: What I Want My Loved Ones to Know.
You dont need a solicitor to write an Advance Decision. As long as it meets certain requirements then its a legally binding refusal of treatment.
