A Student Record Name change may be processed either in person, at the Office of the University Registrar, 2nd Floor Schmitz Hall, or via email to registra@uw.edu. International students on F and J visas should review the ISS Name Change process before submitting a Student Record Name change request.
To request a name change, send a completed and signed Name Change form with legal documentation to the Registrars office at 141 Uptown East, University of Arkansas, Fayetteville, AR 72701. Proper documentation is a marriage license, divorce decree, birth certificate or court order.
How to file for a name change. In most states, your local court clerk at your county courthouse can provide you with the legal forms, legal documents, name change request form, or name change document with the basic steps you need to request a name change. You must file the original completed documents with the court.
Log in with your 10-digit UW campus ID and PIN, or by verifying four digits of your Social Security Number and a course you took while at UWMadison. Once logged in, find the Academic Records box and click Request My Official Transcript.
Log into Identity. UW and follow the directions to set your Preferred Name with the University. You may choose a different first, middle, and/or last name as your Preferred Name.
You are required to provide complete and accurate information when self-reporting. Although official transcripts are not required at the time of application, you will be required to provide a final official high school transcript if you are admitted and plan to enroll at the UW.
You may update your addresses via MyUW. Another option is to submit a Student Record Change or Correction Form. International students: You may change your local address and mailing options through MyUW. However you must contact International and Student Scholar Services to change your permanent address.
All that was required was a trip to the registrars office and documentation that I had changed my name. Its similar to any other name-change situation. There will be required documentation needed and a form filled out. The registrar will help you easily.
