Ontarios health care system is becoming increasingly integrated through provincial electronic health record (EHR) products and services. OntarioMD (OMD) is integrating these provincial assets with electronic medical records (EMRs) used by clinicians in the community.
Disclosure with consent Except for limited circumstances specified in the HIA, a custodian must get your written consent before releasing information to a third party, such as a family member, lawyer, or insurance company. Consent allows for disclosure to anyone for any purpose, ing to the terms of the consent.
A fee of $30.00 shall apply to patient, SDM and lawyer requesters. This includes an initial set amount for photocopying and/or printing of a record and shall include pages 1-20. This fee may also be charged when a search does not yield a return of a patients record.
Contact the custodian of your health records, such as a doctor, clinic or hospital, to request access. The custodian might ask you to make a formal request, in writing. You can write a letter or use this Request to Access Personal Health Information Form.

The data collected includes administrative and demographic information, diagnosis, treatment, prescription drugs, laboratory tests, physiologic monitoring data, hospitalization, patient insurance, etc. Individual organizations such as hospitals or health systems may provide access to internal staff.
Add your health records On your iPhone or iPad, open the Health app. If youre on your iPhone, tap the Summary tab, then tap your profile picture. Under Features, tap Health Records, then tap Get Started. Youre prompted to allow the Health app to use your location to find hospitals and health networks near you.
Adult patients: 10 years from the date of the last entry in the record. Patients who are children: 10 years after the day on which the patient docHubed or would have docHubed 18 years of age.
