Use Forms to add a survey or form to your Sites page Go to Formsand open your form. Click Send. Next to Send via, click Embed . Click the HTML and click Copy. Paste the HTML into your site or blog.
Edit the appearance settings and click Next to control the survey experience or add restrictions to the survey. Click Get Embed Code. Copy and paste the Embed code on a page in your site. When embedding a survey, paste the script into your HTML wherever you want the survey to appear on your page.
To embed your survey into a webpage, simply follow the steps described here: Select your survey, and go to the share step. Copy the generated Iframe code which is unique for your survey. Then go to your websites admin panel or to the HTML source of your site or blog, and paste the iFrame code in it, where desired.

Embed a survey with Google Forms To start, create a survey in Google Forms. Youll need a Gmail account to complete the process. Once youve drafted a survey, embed it into an email by clicking the Send button in the top right. Check the Include form in Email box at the bottom of the Send form.
To embed a like or dislike question in your email template, select the question in the Select Content dropdown (1). To select an individual or a list of participants and continue with the wizard to configure merge fields and the survey invitation link options, click Next.
How to embed a survey in a website Start by creating a survey if you havent already. Next, go to the Publish tab in Jotforms Form Builder. Click Embed in the left sidebar. Click the green Copy code button. Paste the code into your web page where you want the survey to display.
