hi Brendan Lou Patton from Meijer met Malcolm at the beginning of pretty much every medical malpractice case that I handle my clients oftentimes ask me will they change my medical records patients are understandably very nervous because they dont have control of the records the hospitals the doctors and the nurses do about their records getting changed after the fact and ruining their otherwise righteous medical malpractice case well I have good news well it is still possible for doctors and nurses and risk managers at hospitals to change your medical records theres a lot less incentive to do so because theres a lot more accountability in today and thats due to technology specifically something called audit trails so nowadays unlike the old days all of your medical records are electronic there are electronic programs that keep track of all of your different medical records in the old days they were paper records and doctors and nurses could easily go back and cross things out or

If you want to have a mistake fixed, follow these steps: Step 1: Contact your provider. Contact your providers office and find out what their process is for updating or correcting your health record. Step 2: Write down what you want fixed. Step 3: Make a copy of your request. Step 4: Send your request.
Making Your Request Contact the hospital or your payer to ask if they have a form they require for making amendments to your medical records. If so, ask them to email, fax, or mail a copy to you.
When an error is made in a medical record entry, proper error correction procedures must be followed. Draw line through entry (thin pen line). Initial and date the entry. State the reason for the error (i.e. in the margin or above the note if room). Document the correct information.
