If you want to add an image to your survey you can use the green bar (Change Question Type) at the top right of your page to insert a graphic. You can also insert a graphic in the text of a question by using the Rich Content Editor and clicking on the graphic icon (top row, second from left).
You can upload an image from your computer into a question text, answer choice field, or Image A/B Test field. To add an image: Add your question or image type by dragging and dropping it from the Build section of the left side bar onto your survey.
