Edit a signed SOAP or Simple Note An encounter cannot be edited or deleted after it has been signed. The act of signing a chart note renders the note a legal document. However, you can add an amendment/addendum to the note.
Unsigned encounters can be deleted from the Actions menu of the Encounter you wish to delete. Signed encounters can only have an addendum added and cannot be deleted.
Information Navigate to the signed encounter in the patients chart. Click Edit next to the Encounter type header in the Addenda pane (see Graphic 2). Select the new code from the Encounter type drop-down (see Graphic 3). Add any additional comments in the Addendum Notes field and then press Accept (see Graphic 4).
Navigate to Display settings in the top left corner of the patient encounter as seen in Graphic 1 below. 2. In the Encounter display settings pop-up modal, use the checkboxes to choose which sections you would like shown when charting. You may also opt for specific sections to be minimized by default.
Use the drop-down to find Encounters and click on the encounter you wish to view (see Graphic 9). Navigate to Charts using the left navigation bar, highlighted in Graphic 3, below. From the Summary tab (see Graphic 3) or the Timeline tab (see Graphic 4), click the orange New Encounter button to create a note.
Information Chart notes that have been signed cannot be edited or deleted. Navigate to the patient Summary and click on the encounter to which you would like to add an addendum. When in a signed encounter, you will only have two options at the top right corner, Print and Add addendum.
Editing or deleting signed notes Signed notes can be deleted, but a record of the deletion will be retained and displayed on the patients note summary view.

If you would like to create a subgroup of patients you would like to monitor, click the Star icon next to the patient name from their Profile section. This action will add this patient to the Starred Patients list. Click the Star again to remove the patient from the list.
