In this tutorial, Neil Winger, a primary care physician affiliated with the UCLA Health Ethics Center, discusses advance directives. An advance directive is a legal document that specifies an individual's preferences for medical treatment when they are unable to communicate their wishes. This situation may arise if a patient is too ill or unconscious, preventing effective shared decision-making with their healthcare providers. It is crucial for patients to designate a trusted person, such as a spouse, sibling, or friend, to make medical decisions on their behalf in such circumstances. The video emphasizes the importance of having advance directives in place to ensure that treatment aligns with the patient's desires.

Advance directives have limitations. For example, an older adult may not fully understand treatment options or recognize the consequences of certain choices in the future. Sometimes, people change their minds after expressing advance directives and forget to inform others.
An Advance Care Plan stating your wishes is not legally binding, but anyone who is making decisions about your care should take it into account. In some cases, it may not be possible to follow your wishes. For example, you may prefer to be cared for at home, but you develop a new symptom that cant be managed at home.
Advance care planning (ACP) is the term used to describe the conversation between people, their families and carers and those looking after them about their future wishes and priorities for care.
Witness. Your witness can be anyone over the age of 18. If possible, it is a good idea to make sure your witness is not a close relative, partner, anyone who will inherit your money or property after your death, or your attorney (the person appointed to make decisions on your behalf through a Lasting Power of Attorney)
Texas Health and Safety Code, Section 166.45-51 states that, in Texas, euthanasia is not condoned or authorized. Further, any act or omission that works toward a persons death is also illegal, except for allowing a person to die naturally without interference.
You and a witness have to sign the personal directive to make it a legal document. Make sure you read the form about who can or cannot sign the form as a witness.
Review the advance care plan whenever treatment or support is being reviewed, while the person has capacity. Consider whether it would be helpful to involve a healthcare professional. Make any changes requested, including to any copies.
The person should give a copy of the Advance Directive to her Agent if the she has one. The Agents job is to make sure that the persons decisions are known and followed. To do this, the Agent must have a copy of the Advance Directive that appoints her as the persons Agent.
