Chart notes that have been signed cannot be edited or deleted. However, you can add an amendment/addendum to the note which allows additional information to be recorded in the medical record.
1. Press the Actions button in the upper right-hand side of the encounter and select Delete encounter from the drop-down menu (See Graphic 1). 2. Press the Delete button on the Delete encounter confirmation modal (see Graphic 2).
Information Chart notes that have been signed cannot be edited or deleted. Navigate to the patient Summary and click on the encounter to which you would like to add an addendum. When in a signed encounter, you will only have two options at the top right corner, Print and Add addendum.
Edit a signed SOAP or Simple Note An encounter cannot be edited or deleted after it has been signed. The act of signing a chart note renders the note a legal document. However, you can add an amendment/addendum to the note.
Deleting a Patient Encounter Note From the Assignment Manager menu, select Patient Assign. In the Assign section of the toolbar, click Patients. Click the Patient Encounter Note icon at the end of the patients row. View the note or notes. Clear all text within the note box. Click Save.
Rachel W. For HIPAA purposes, a SOAP Note that has been saved or saved as a draft cannot be deleted. A copy of each saved note is stored in the SOAP Notes tab of the Clients record for viewing and editing.
