Select from the list of appointments, and double-check the chart note details at the bottom of the screen. You can use the Find by menu to search for the chart note by date or other details. When you are certain you have found the chart note you need to delete, click Delete.
If you decide you no longer want a My Health Record, call the help line on 1800 723 471 and ask to have your record cancelled. You can also log in through myGov to cancel your My Health Record. If you cancel your My Health Record, all information in the record will be permanently deleted.
To delete a note: With the correct note open, click the Delete Icon.
To delete an existing note: In the section labeled Notes and Files for this Patient, click the Edit. icon for the note you want to delete. Click the Delete Note link in the bottom right corner of the note. A warning dialog will appear to alert you that the note will be permanently deleted. Click the Delete button.
