Office.com is how we are going to get to Forms. So once you navigate to that website, youre going to see a website load and you have two primary actions. You can either get Office or you could sign in. If you have an account or even if you dont have an account, what were going to do is were going to click on Sign in. To update your address Go to to update your NPI address.
Take care of your NPI. You are responsible for updating NPI-related information with NPPES, the National Plan and Provider Enumeration System. If you are a covered health care provider, you must report any changes to any of the information that was furnished to obtain your NPI within 30 days of the change.
Once you have been assigned an NPI, you must furnish updates to its data within 30 days of any changes. You may received notices about the NPI from other health plans with which you do business, but remember, you need apply only once for an NPI.
Updates can be made by mailing a Paper Application/Update Form available for download: NPI Application/Update Form. In Section 1A Reason For Submittal of this Form, select the Change of Information box.
Go to to update your NPI address. 2. If you forgot your User ID or Password on the NPPES (NPI) website: Click on the Forgot User ID or Password? button and follow the instructions. Once you have reset your User ID or password, log out of the website.
Updates can be made online by accessing and completing the steps below: NOTE: The following changes cannot be completed online and require a Paper Application/Update Form: Changes to Date of Birth, Changes to Social Security Number, Reactivation of an NPI and Deactivation of an NPI.
