A clients initials are considered to be identifying for the purposes of determining if a given piece of information is PHI under HIPAA, because they are derived from names. Even though most people couldnt identify a client from just their initials, some people can.
If patient initials are maintained in a data set that does not contain health information (i.e., initials, surname, and telephone number), none of the information in the data set is PHI because they do not relate to the individuals health information.
