Basic Incident Information the names of any witnesses. the exact location and/or address of the incident. the exact time and date of the occurrence. a detailed and clear description of what exactly happened.
An incident report should be objective and supported by facts. Avoid including emotional, opinionated, and biased statements in the incident report. It should provide both sides of the story and should not favor one side.
The key tasks to mention in the workflow includes notification of the incident, identification of responsible, interviews, investigation and analysis, conclusion, sharing learnings and implementation.
It should include: the names and positions of the people involved. the names of any witnesses. the exact location and/or address of the incident. the exact time and date of the occurrence. a detailed and clear description of what exactly happened. a description of the injuries.
OOTB, any user with the Admin role can delete records. This is controlled through an ACL. If youd like other roles to be able to delete any incident record, you could configure a new ACL based upon this ACL.
How to Delete an Incident Open the incident form you want to delete. From the form toolbar on the right side, hover over the Form Options icon and select Delete Incident. Enter a reason for deleting the incident and click Delete Incident.
