Which record would cause a concern during a RADV audit? A record with conflicting documentation about a diagnosis included in RADV. Rational: Records supporting reported diagnosis should NOT have conflict information.
Signature Logs Type or print the providers credential. The physician (provider) should sign his/her legal signature (full name, including credential). Under Actual Chart Signature, the provider should indicate all possible ways that he/she would sign the medical record (initials, first initial/last name, etc.).
Most notably, the Final Rule: (1) allows CMS to extrapolate RADV audit findings beginning with Payment Year (PY) 2018; and (2) does not include a Fee-For-Service (FFS) adjuster in RADV audits, which was previously contemplated as a method of equalizing payment errors between FFS Medicare and Medicare Part C, and viewed
CMS does so based on diagnosis codes that MAOs submit for each enrollee based on the enrollees encounters with healthcare providers. CMS has conducted annual RADV audits of a limited number of MA plans.
About the Program The Medicare Risk Adjustment Validation Program is CMS primary way to address improper overpayments to Medicare Advantage Organizations (MAOs). During a RADV audit, CMS confirms that any diagnoses submitted by an MAO for risk adjustment are supported in the enrollees medical record.
Submissions must include the: Correct beneficiary as provided on the CMS RADV coversheet. Acceptable risk adjustment provider type, source, and physician specialty providing the face-to-face encounter. Dates of service within the data collection period under review.
RADV Audits. CMS conducts two types of RADV audits: Annual national level audits to estimate the national MA improper payment rate. Contract-level RADV audits to identify and recover improper payments from.
RADV audits are the main corrective action for those improper payments. Through RADV audits, a sample of beneficiary medical records are provided by MAOs, and CMS reviews those records to verify that diagnoses reported for risk adjusted payments are accurate and supported in the medical record.
