During a Pap test, a tool called a speculum holds the vaginal walls apart. A sample of cells from the cervix is collected using a soft brush and a flat scraping device called a spatula (1 and 2). The cells are placed in a bottle that contains a solution to preserve them (3).
Wash smears in running water for 1 minute. NOTES: This step removes spray-fixative and the timing is for use with water soluble fixatives. Initial washing is important and must be thorough to expose the cell to staining. If slides have been fixed in 95% ethanol, timing may be shortened to a few seconds.
The universal stain for cytological preparations is the Papanicolaou stain. May-Grnwald Giemsa Stain is one of the common Romanwsky stains used in cytology. It is useful for studying cell morphology in air-dried smears.
Staining Prepare fresh working Giemsa stain in a staining jar, ing to the directions above. Pour 40 ml of working Giemsa buffer into a second staining jar. Place slides into the working Giemsa stain (2.5%) for 45-60 minutes. Remove thin smear slides and rinse by dipping 3-4 times in the Giemsa buffer.
Saline solution may be used for moistening. The lubricant does not dissolve during processing of the slide. It often stains dark purple and may hide numerous cells. Before scraping, the wooden spatula should be dipped in saline to moisten it and prevent too rapid dehydration of the scraped cells.
Staining techniques The Papanicolaou stain (PAP) requires immediate fixation in alcohol. Nuclear chromatin patterns are easily discerned and the degree of maturation/keratinisation of squamous cells is readily appreciated. Mucin may not be seen so easily. May-Grnwald Giemsa (MGG) staining is used on air-dried smears.
