Assessing pallor in a child as per IMNCI guidelines: Compare the colour of the childs palm with your own palm and with the palms of other children. If the skin of the childs palm is pale, but has some pink areas, the child has some palmar pallor.
An indirect light source may not help in the visualization of inside of the body cavity. The nurse should inspect each area for size, shape, color, symmetry, position, and abnormality to detect any changes. Checking for side-to-side symmetry ensures that the inspected area has no deviation.
In those with light skin tones, cyanosis will present as a bluish/purple hue. In patients with naturally yellow toned skin, cyanosis may cause a grayish-greenish appearance. In those with darker skin tones, cyanosis may be trickier to assess and may be observed as grey or white.
Recommendations for assessing dark-skinned patients When assessing a patients skin, use natural light or a halogen lamp rather than fluorescent light, which may alter the skins true color and give the illusion of a bluish tint. Skin color is particularly important in detecting cyanosis and staging pressure ulcers.
When the oxygen level has dropped only a small amount, cyanosis may be hard to detect. In dark-skinned people, cyanosis may be easier to see in the mucous membranes (lips, gums, around the eyes) and nails. People with cyanosis do not normally have anemia (low blood count).
Here are conditions and techniques to use in the assessment of darkly pigmented skin: CyanosisInspect the conjunctivae, palms, soles, oral mucosa and tongue. PallorInspect the sclera, conjunctivae, oral mucosa, tongue, lips, nail beds, palms and soles.
There are five key areas to note during a focused integumentary assessment: color, skin temperature, moisture level, skin turgor, and any lesions or skin breakdown.
Pallor may be difficult to detect in dark toned skin and may present as ashen or grey. In brown toned skin the skin will present more yellowish in colour. An alternative method for identifying pallor in darker skin tones can be assessing the palmer surface which can appear paler.
