In dark-skinned patients, skin color changes may not be apparent in nailbeds, palms of the hands, and other exposed areas.
In dark-skinned people, cyanosis can be best assessed by examining the palms of the hands, soles of the feet, tongue, conjunctivae, or the buccal mucosa. In light-skinned people, the nailbeds and the area around the lips can be used.
Skin color matters in health care. Clinicians make decisions based on color assessments multiple times each day as they gauge tissue perfusion and assess for jaundice, pallor, cyanosis, and the blanch response.
Pallor may be difficult to detect in dark toned skin and may present as ashen or grey. In brown toned skin the skin will present more yellowish in colour. An alternative method for identifying pallor in darker skin tones can be assessing the palmer surface which can appear paler.
Skin color may be described as constitutive or as facultative. Constitutive skin color represents an individuals baseline, or the color of skin that has not been altered by sun or other types of ultraviolet (UV) exposure; one example of a constitutive skin site in most people is the upper inner arm.
Recommendations for assessing dark-skinned patients When assessing a patients skin, use natural light or a halogen lamp rather than fluorescent light, which may alter the skins true color and give the illusion of a bluish tint.
