The patellar tendon graft (PTG) has always been the gold standard for anterior cruciate ligament (ACL) reconstruction. Yet, most orthopedic surgeons prefer hamstring grafts for younger athletes and cadaver grafts for older patients.
If nothing is done, the ACL injury may turn into chronic ACL deficiency. Your knee may become more and more unstable and may give out more often. The abnormal sliding within the knee also can hurt cartilage. It can trap and damage the menisci in the knee and can also lead to early osteoarthritis.
The patellar tendon and hamstring autografts are the most common choices for ACL reconstruction. The patellar tendon runs from the knee cap (patella) to the lower leg bone (tibia). Surgeons have the most experience with this autograft and it is the most widely used.
Injuries to the infrapatellar branch of the saphenous nerve are not uncommon in ACL-R. However, there are few case reports also describing injuries to the saphenous nerve, the common peroneal nerve and the sciatic nerve.
The Procedure It will require an incision to remove a tendon if you are undergoing an autograft, in which a tendon from another part of your body is inserted in your knee. Several small punctures are made for an arthroscope – a thin, flexible scope that allows the surgeon to see in your knee – and surgical instruments.
You will feel bone pain since this surgery involves some drilling into bone and you will feel the general pain and fullness due to swelling in your knee. You may also have pain in your throat as most people who undergo a general anesthetic have a tube put into their throat.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
A torn ACL is very painful and can debilitate a person for several months and perhaps for life, although recovery for some is possible.
Current pain relief standards for ACL repair include a femoral nerve block, where anesthesiologists inject freezing around the nerve in the groin to interrupt pain -- similar to a dental freeze. The new technique, called an adductor canal block, freezes nerves at the mid-thigh instead.
