Description cardiogenic shock. acute heart failure. right ventricular infarction. mechanical complications, such as left ventricular (LV) septal rupture, LV free wall rupture, mitral regurgitation, and LV aneurysm. electrical complications, such as ventricular arrhythmias, atrial fibrillation, and atrioventricular block.
Ventricular septal rupture and free wall rupture May develop as early as 24 hours after myocardial infarction but often presents 2-7 days afterwards.
In a myocardial infarction transmural ischemia develops. In the first hours and days after the onset of a myocardial infarction, several changes can be observed on the ECG. First, large peaked T waves (or hyperacute T waves), then ST elevation, then negative T waves and finally pathologic Q waves develop.
Myocardial ischemic-like ECG changes include ST-segment deviations, T wave inversion, and Q-waves. The earliest manifestations of myocardial ischemia typically involve T waves and the ST segment.
The most commonly encountered mechanical complications are acute mitral regurgitation secondary to papillary muscle rupture, ventricular septal defect, pseudoaneurysm, and free wall rupture; each complication is associated with a docHub risk of morbidity, mortality, and hospital resource utilization.
When the patient is suffering acute inferior STEMI a right-sided 12-lead ECG can help to identify right ventricular infarction.
Complications Cardiogenic shock: Cardiogenic shock complicating anterior MI is associated with higher hospital mortality when compared to inferior MI.[22] Left ventricular dysfunction. Left ventricular mural thrombus. Ventricular septal rupture. Free wall rupture. Pericardial effusion from free wall rupture.
Conclusion: The diagnosis of STEMI by synthesized 18-lead ECG is useful to identify the site of infarction in patients with infarction of the right ventricular wall (supplied by the RCA) or posterior wall of the left ventricle (supplied by the LCX), which often fail to be diagnosed by the standard 12-lead ECG.
All patients with inferior wall myocardial infarction should have a right-sided ECG.
Ventricular free wall rupture. VFWR is the most serious complication of AMI. VFWR is usually associated with large transmural infarctions and antecedent infarct expansion. It is the most common cause of death, second only to LV failure, and it accounts for 15-30% of the deaths associated with AMI.
