Reference values The 99th percentile cutoff for cardiac troponin T (cTnT) is 0.01 ng/mL. The reference range for the high sensitivity troponin T is a normal 14 ng/L, borderline of 14-52 ng/L, and elevated of 52 ng/L.
For hs-cTnT, abundant data suggest that the LOD (5 ng/L) is appropriate. For hs-cTnI (Abbott ARCHITECT), recommendations from the available data are mixed. A cutoff set at the LOD (2 ng/L) appears safer, although the data of Neumann et al. (9) suggest that the more liberal cutoff of 3 ng/L may suffice.
For example, the normal range for troponin I is between 0 and 0.04 ng/mL but for high-sensitivity cardiac troponin (hs-cTn) normal values are below 14ng/L. Other types of heart injury may cause a rise in troponin levels. These include: Atrial fibrillation.
Troponin T, High Sensitivity 1-13 ng/L Troponin T, High Sensitivity is below the upper reference limit (14 ng/L) and results are not consistent with myocardial infarction or injury, provided the specimen was collected more than 6-hours from the onset of symptoms.
The results of this study suggest that an acceptable cutoff for cTnI to rule-out AMI patients who have CHF and or ESRD is 0.2 ng/mL which is four times higher than the original, but has comparable specificity, sensitivity, PPV and PPV to the total patient population.
Currently, 14 ng/L is considered as the cutoff value for a positive Hs-troponin result when evaluating patients with NSTEMI. This value is highly sensitive but lack specificity. When conducting a ROC curve on our data a Hs-troponin value of 14 ng/L had a sensitivity of 99.9% with an only 4% specificity.
Access High Sensitivity Troponin I (Beckman Coulter) The Access High Sensitivity Troponin I assay has a recommended 99th centile cut-off point of 17.5 ng/l for the whole population, 11.6 ng/l for females and 19.8 ng/l for males, with a CoV of 10%. The assay can detect troponin I in 50% of the reference population.
Having a result between 0.04 and 0.39 ng/ml often indicates a problem with the heart. However, a very small number of healthy people have higher-than-average levels of troponin. Due to this, if the result is in this range, a doctor may check for other symptoms and order further tests before making a diagnosis.
