Surgical treatment is recommended for individuals with a grade 3 or complete ACL tear. Surgical options may vary based on the type of ACL injury, whether the patient has open or closed growth plates, and the type.
Four distinct repetitive patterns of ACL match injuries were identified: (1) non-contact pressing ACL injury (n=9), (2) indirect contact parallel sprinting and tackling ACL injury (n=7), (3) direct contact knee-to-knee ACL injury (n=6) and (4) non-contact landing ACL injury (n=4).
The median scale value was 4.5 (lower quartile 3.0; higher quartile 7.0). Based on these quartiles, a score 4 was considered to be an injury of mild severity (grade I), a score between 4 and 7 was defined as moderately severe (grade II) and a score 7 displayed the most severe cases of ACL injuries (grade III).
Readiness for sport was based on criteria for the ACL-RSI scale score (low threshold, 56 points or greater; high threshold, 75 points or greater) and hop tests (90% or greater limb symmetry).

The ACL - Return to Sport after Injury (ACL-RSI) scale, published in 2008 in Physical Therapy in Sport, was developed in order to measure athletes emotion, confidence, and risk appraisal when returning to sports after an ACL injury and/or reconstructive surgery.
For the ACL-RSI, a score of 76.6 points was used as a cut-off or passing score, since this score has been reported to have maximal sensitivity for discriminating between patients who suffer a second ACL injury and patients who do not.
Reduced activities. Without an ACL, the knee is unlikely to support aggressive landing, cutting and pivoting. Living with a torn ACL may mean limiting participation in sports, work and activities that cause the knee to swell, give way or feel unstable.
