The ACL measures 31-38 mm in length and 10-12 mm in width, with the anteromedial bundle (6-7 mm) slightly thicker than the posterolateral bundle (5-6 mm) 5.
The ACL was bluntly separated into the two bundles. Note that the AM bundle attaches on the cylindrical surface of the femoral intercondylar notch around its proximal/posterior outlet. PCL: Posterior cruciate ligament, AMB: Anteromedial bundle, PLB: Posterolateral bundle.
The anterior cruciate ligament (ACL) consists of two major fiber bundles, namely the anteromedial and posterolateral bundle. When the knee is extended, the posterolateral bundle (PL) is tight and the anteromedial (AM) bundle is moderately lax.
The bulk of the fibers of the anterior cruciate ligament (ACL) is taut in maximal extension, while that of the posterior cruciate ligament (PCL) is taut in the intermediate positions and in maximal flexion.
While the anteromedial bundle is the primary restraint against anterior tibial translation, the posterolateral bundle tends to stabilize the knee near full extension, particularly against rotatory loads.
The AM has a greater impact on the APT than the PL in all knee joint flexion angles. The PL does not resist the rotational stability more than the AM. The rotational stability is better controlled by both bundles of ACL as compared to one bundle of the ACL.
Thus, we concluded that both ligaments function to prevent valgus instability, but that the anatomical reasons for their function are different. The MCL prevents valgus instability by stopping an opening in the medial joint space. The ACL, on the other hand, prevents the internal rotation of the tibia.
