Cardiac troponin I: the gold standard in acute myocardial infarction diagnosis.
An MI is diagnosed when 2 of the following criteria are met: Symptoms of ischemia. New ST-segment changes or a left bundle branch block (LBBB) Presence of pathological Q waves on electrocardiogram (ECG)
Answer: The studies by Reichlin and colleagues found that an initial troponin T value less than 14 ng per L that increases by less than 4 ng per L in the next two hours (or a value less than 13 ng per L that increases by less than 3 ng per L in the next hour) effectively rules out AMI.
We have proposed a cut-off value of 100 ng/L as diagnostic for acute myocardial injury in the absence of end-stage renal disease and AMI in ischemic patients. If one extrapolates this cut-off to chest-pain patients, it accurately predicts AMI with a positive predictive value of 90% and the specificity 99%.
The results of this study suggest that an acceptable cutoff for cTnI to rule-out AMI patients who have CHF and or ESRD is 0.2 ng/mL which is four times higher than the original, but has comparable specificity, sensitivity, PPV and PPV to the total patient population.
For troponin concentrations 0.40 ng/mL and higher, the underlying cardiac injury is usually a myocardial infarction. Troponin concentrations of 0.04-0.39 ng/mL require serial troponin measurements and clinical correlation to interpret, as further described in the guidelines.
Very high levels of troponin typically indicate that a person has had a heart attack, which can occur if the blood supply to some of the heart muscle suddenly becomes blocked.
The manufacturers upper reference level for troponin T is 14 ng/L. Based on the meta-analyses, cut-off values for troponin in patients with renal impairment with myocardial infarction was 42 ng/L for troponin I and 48 ng/L for troponin T. For patients on dialysis the troponin T cut-off is even higher at 239 ng/L.
