Since z-scores go out to infinity, there is no single cutoff. A lot depends on your risk. Traditionally, two standard deviations (z score of -2 or +2) is used, that represents a spread of 95% of the data IF your data are normal. Tchebychev inequality gives you a worst case of 75%( 1 - 1/ z squared).
To calculate the cutoff value, multiply the total score or value by the percentile. Then, subtract the rate of rejection from 1. Divide the first result by the second result to get the cutoff value.

Statistical Analysis: In statistical analysis, a cutoff point is a designated point of division in a set of data. For example, in hypothesis testing, a significance level (commonly 0.05) is a cutoff point used to reject or fail to reject a null hypothesis.

So, any predicted probability of 0.5 or greater is classified as having the outcome and any predicted probability below 0.5 is classified as not having the outcome.
Imagine a binary classification problem. Lets say I have 800,000 predicted probabilities stored in predtest . I define a cutoff as any value in predtest such that the values that are greater than or equal to cutoff are assigned the value 1 and the values that are smaller than cutoff are assigned the value 0.
