For BCC with characteristics matching this study, overall adequate microscopic excision depths are proposed: superficial, 1.0 mm; superficial combined with nodular, 2.0 mm; nodular, 3.0 mm; and aggressive, 3.0 mm.
(MAR-jin) The edge or border of the tissue removed in cancer surgery. The margin is described as negative or clean when the pathologist finds no cancer cells at the edge of the tissue, suggesting that all of the cancer has been removed.
The recommended surgical margin is four millimeters, and adequate tumor removal has been demonstrated in 98% of cases of non-sclerodermiform BCCs, with less than two centimeters in diameter. However, in places such as the face, functional and cosmetic issues often do not allow surgical excision with such margin.
Margin width should be 1 cm for melanomas 1 mm thick, 1 or 2 cm for melanomas 1 to 2 mm thick, and 2 cm for melanomas 2 mm thick. The margin width for wide local excision of a melanoma in situ should be 5 mm. Standard wide local excision margin recommendations also apply to melanomas of the skin of the digits.
During a standard surgical excision, a doctor removes the entire tumor with a border of healthy tissue. He or she closes the incision with stitches, and the tissue is sent to a laboratory, where a dermatopathologist examines it under a microscope to confirm that the entire tumor has been removed.
Sacrificing tendons, blood vessels, and nerves to obtain adequate margins may result in docHub functional loss. It is often difficult to obtain adequate margins in the resection of soft tissue sarcomas of the hand, and the rate of positive margin after surgery is reported to range from 11.3% to 36.4% (31-33).
Cancer Council Australias clinical guidelines suggest 23 mm surgical margins for low-risk tumours and margins 5 mm for high-risk tumours.
The European Dermatology Forum (EDF) guidelines on surgical excision margins of BCC recommends 3 to 4 mm peripheral margins for low-risk BCC, and 5 to 10 mm peripheral margins for high-risk BCC.
