During ACL reconstruction, the torn ligament is removed and replaced with a band of tissue that usually connects muscle to bone (tendon). The graft tendon is taken from another part of your knee or from a deceased donor.
Most people have some surgery-related pain and discomfort for the first week or so. Not surprisingly, pain from knee surgery decreases with time. By the end of a week or two at the most, you should have very little discomfort. Swelling and bruising are also relatively common, and like discomfort, they're temporary.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
By the end of the second or third week, patients usually walk without crutches. Sessions with a physical therapist usually begin seven to 14 days after surgery. During physical therapy, weight bearing is allowed if you did not have a meniscus repair.
The procedure usually takes less than two hours. It will require an incision to remove a tendon if you are undergoing an autograft, in which a tendon from another part of your body is inserted in your knee.
The ACL footprint is positioned at the space formed by the resident's ridge, the cartilage margin of the lateral femoral condyle, and Blumensaat's line. The center of this footprint could serve as a reference point for femoral tunnel formation during anatomical single-bundle ACLR.

The patellar tendon graft (PTG) has always been the gold standard for anterior cruciate ligament (ACL) reconstruction.
Recently, Lubowitz et al. 13 have elegantly shown that placing the guide at a 60° angle to the longitudinal axis of the femur in the coronal plane and at a 20° angle to the transepicondylar axis in the axial plane most closely replicates the ideal femoral socket diameter, area, length, and angle.
