A positive test with a high AFP suggests a birth defect like spina bifida. That's typically a result of 2.5 times or more than the "average" level of AFP you'd expect to see at that point in your pregnancy. A positive test with low AFP could mean a problem like Down syndrome or Edwards syndrome.
Normal and Critical Findings Between 15 and 20 weeks, AFP levels usually range between 10 ng/ml to 150 ng/ml. Adult blood levels greater than 200 ng/ml in patients with liver cirrhosis strongly indicate hepatocellular carcinoma.
Higher than normal AFP levels may mean your baby has an increased risk of having a neural tube defect, such as: Spina bifida, a condition in which the bones of the spine don't close around part of the spinal cord. Anencephaly, a condition in which the brain does not develop properly.
In the United Kingdom as a whole, women with serum-A.F.P. levels above 2-5 times the normal median at 16-18 weeks of gestation will have an approximately 1-in-20 chance of having a fetus with open spina bifida; the risk of having any N.T.D. will be approximately 1 in 10.

False-positive results may be because the mother is carrying twins, inaccurate dates, a low birth weight infant, a large placenta or a sample that was contaminated. Abnormal results are also associated with the fact that the baby may be a girl.

AFP is measured in nanograms per milliliter (ng/mL). An AFP level between 0 ng/mL to 40 ng/mL is normal for adults. An extremely high level of AFP in your blood—greater than 400 ng/mL—could be a sign of liver tumors.
High AFP levels may also mean that you are having more than one baby. You may also get a false-positive result. That means that your AFP results aren't normal, but your baby is healthy. If your AFP test results aren't normal, you will likely have more tests to help make a diagnosis.
Lower than normal AFP levels may mean your baby has a genetic disorder such as Down syndrome, a genetic disorder that causes intellectual disabilities and health problems.
