Injuries to the infrapatellar branch of the saphenous nerve are not uncommon in ACL-R. However, there are few case reports also describing injuries to the saphenous nerve, the common peroneal nerve and the sciatic nerve.
Pathology. An anterior cruciate ligament graft tear is a partial or complete discontinuity of the graft and accounts for graft failure.
Injury to the infrapatellar branch of the saphenous nerve (IPBSN) is common after arthroscopic ACL reconstruction with hamstring tendon autograft, as reported in up to 88% of the cases.
The signs of ACL graft failure can include swelling, pain within the knee, locking within the knee, a mechanical block (which can be due to a bucket-handle tear of the meniscus), lack of full motion, and difficulty with twisting, turning, and pivoting.
If the tibia, or shinbone, has more movement, or if the ligament is loose compared with the other knee, the anterior drawer test is considered to be positive.
Current pain relief standards for ACL repair include a femoral nerve block, where anesthesiologists inject freezing around the nerve in the groin to interrupt pain -- similar to a dental freeze. The new technique, called an adductor canal block, freezes nerves at the mid-thigh instead.
When performing reconstruction of the ACL, the major complications that can arise include missed concomitant injuries, tunnel malposition, patellar fracture, knee stiffness, and infection. We review the complications that can occur as a result of errors made before, during, and after surgery.
The test is positive when there is an excessive anterior movement of the foot and a dimpling of the skin on both sides of the Achilles tendon when compared to the uninjured foot. A positive test also implies a rupture of the anterior talofibular joint. No psychometric properties have been reported for this ankle test.
How Do You know if You Tore your ACL After Surgery? To get an idea if you tore your ACL after surgery, you may hear a popping or cracking sound, which is usually followed by pain that is mild or severe. Swelling or tenderness is also common after an ACL injury, and there may be some redness around the knee.
Most people will experience a return in some or part of their sensation. This is probably because the nerve was simply stretched through the surgery. Or other nerves have grown and compensated for the other. If the nerve is purely stretched, experts would suggest that the nerve should recover in 6-12 weeks.
