Answer: This clinically indicates non-ischemic myocardial injury. Although there were positive troponins, they were relatively flat, and the cardiologist indicated that the troponin elevation is likely due to non-ischemic causes - a combination of CKD and “troponin leak” from cardiac strain due to CHF.
For troponin concentrations 0.40 ng/mL and higher, the underlying cardiac injury is usually a myocardial infarction.
Increased troponin levels may also be due to: Abnormally fast heartbeat. High blood pressure in lung arteries (pulmonary hypertension) Blockage of a lung artery by a blood clot, fat, or tumor cells (pulmonary embolus) Congestive heart failure. Coronary artery spasm.
Normally, troponin stays inside your heart muscle's cells, but damage to those cells — like the kind of damage from a heart attack — causes troponin to leak into your blood. Higher levels of troponin in your blood also mean more heart damage, which can help healthcare providers determine the severity of a heart attack.
Because cardiac troponin is a sensitive and specific measure of myocardial necrosis, it is the preferred biomarker for use in the diagnosis of acute MI. Although an elevated troponin indicates myocardial necrosis, it does not always indicate MI.
What Abnormal Results Mean. Even a slight increase in the troponin level will often mean there has been some damage to the heart. Very high levels of troponin are a sign that a heart attack has occurred. Most patients who have had a heart attack have increased troponin levels within 6 hours.
There are two types of troponin that are more detectable after heart muscle damage, which use the letters I and T to tell them apart. Troponin I (cTnI). This kind of troponin is unique to heart muscle. Troponin T (cTnT). Troponin T does exist in other types of muscle, but the amounts are very limited.
Some of the non-cardiac conditions in which elevated troponins are commonly found include: pulmonary embolism, end stage renal disease, Sepsis/ Systemic inflammatory response syndrome (SIRS) and critically ill patient, rhabdomyolysis, physical exercise, burns, drug toxicity and stroke.
