Medicare Secondary Payer (MSP) is the term generally used when the Medicare program does not have primary payment responsibility - that is, when another entity has the responsibility for paying before Medicare.
First dollar coverage is a type of insurance where there is no deductible or copay. The insurance company starts covering costs on the first dollar claimed. First dollar coverage is typically more expensive than a similar deductible plan.
Medicare Secondary Payer (MSP) means another insurer pays for healthcare services first, making that insurer the primary payer. The secondary payer covers some or all of the remaining costs that the primary payer leaves unpaid.
If the disabled person still has insurance from an employer or from a working spouses employer, Medicare is secondary if the employer has at least 100 employees, but primary if it has fewer. When Medicare is secondary, the primary insurer should always be billed first.
Medicare pays first, and the group health plan pays second. If you have employer or union coverage and get Medicare drug coverage, you may lose your employer or union health and drug coverage (for you and your dependents).
The insurance that pays first is called the primary payer. The insurance that picks up the remaining cost is the secondary payer. For example, if you had an X-ray bill of $100, the bill would first be sent to your primary payer, who would pay the amount agreed upon by your plan.
Since Medicare pays first after you retire, your retiree coverage is probably similar to coverage from a Medicare Supplement Insurance (Medigap) policy. Both are likely to offer benefits that fill in some of the gaps in Medicare coveragelike coinsurance and deductibles.
