The physical examination remains a mainstay of orthopaedic diagnosis, and multiple physical examination maneuvers can be used to evaluate a possible anterior cruciate ligament (ACL) injury. Historically, the most commonly used tests are the Lachman test, the anterior drawer test, and the pivot shift test.
The Lachman test is a specific clinical exam technique used to evaluate patients with a suspected anterior cruciate ligament (ACL) injury. The test relies on proper positioning and technique and is regarded as the most sensitive and specific test for diagnosing acute ACL injuries.
MRI is the primary modality to diagnose ACL pathology, with a sensitivity of 86% and a specificity of 95%. Diagnosis may also be made with knee arthroscopy to differentiate complete from partial tears and chronic tears.
Secondary signs include bone marrow edema (secondary to bone contusion) in more than half of ACL tears. The characteristic ACL bone bruising involves the middle third of the Lateral femoral condyle and the posterior third of the lateral tibial plateau.
The highest level of evidence for ACL rehabilitation in this review (moderate certainty) was observed for: (1) neuromuscular electrical stimulation to improve quadriceps strength; (2) open versus closed kinetic chain exercises to be similarly effective for improving quadriceps strength and self-reported function; (3)
An MRI can show the extent of an ACL injury and signs of damage to other tissues in the knee, including the cartilage. Ultrasound. Using sound waves to visualize internal structures, ultrasound may be used to check for injuries in the ligaments, tendons and muscles of the knee.

This can range from minor and inconsequential to severe. Prevention is the first and most effective method for treatment of loss of motion. This is why compliance with post-operative rehabilitation is so vital to the outcome of the procedure, and why range of motion is started immediately post-operatively.
Conclusion. After management of acute ACL rupture with a novel bracing protocol, 90% of patients had evidence of ACL healing on a 3-month MRI (continuity of the ACL). More ACL healing on a 3-month MRI was associated with better knee function and QOL, less passive knee laxity and a higher return-to-sport rate.
