As its a formal letter, include a title before their name, such as Mr., Mrs., Miss, or Ms. Along with their name, consider including their job title as well as the company name and address.
Formal Salutation The format of the salutation depends on the recipients title and gender. If you know the recipients name, use Dear Mr/Mrs/Ms/Dr [Last Name]. If the recipients gender is unknown or irrelevant, Dear [First Name] is acceptable.

The Heading The heading contains the return address with the date on the last line. Sometimes it is necessary to include a line before the date with a phone number, fax number, or e-mail address. Often there is a line skipped between the address and the date.
Most business letters must include a return address (letterhead or your name and address), date, an inside address (receivers name and address), a salutation, body paragraphs, and a closing.
Its best to start your letter with a professional salutation like Dear, and the persons prefix if you know it. For example, if you know the recipient is a doctor, avoid writing Dear Taylor Spenser. Instead, its more appropriate if you write Dear Dr. Spenser.
Before you start writing your business letter, set your font to Times New Roman or Arial, font size 12. Align your text to the left and make sure your writing is single-spaced, with a blank line between paragraphs.
