A living will is a written, legal document that spells out medical treatments you would and would not want to be used to keep you alive, as well as your preferences for other medical decisions, such as pain management or organ donation.
Five Wishes Paper is a traditional printed booklet to complete by hand. It meets requirements in nearly all states. If you live in one of only four states (New Hampshire, Kansas, Ohio, or Texas) you can still use the Five Wishes Paper but may need to take an extra step.
Five Wishes lets your family and doctors know: Who you want to make health care decisions for you when you cant make them. The kind of medical treatment you want or dont want. How comfortable you want to be. How you want people to treat you. What you want your loved ones to know.
A specific and common example of an advance directive is a do not resuscitate order (or DNR), which guides care only if your heart stops beating (cardiac arrest) or you are no longer breathing.
Five Wishes is a set of forms that allows one to name a person to be their health care agent, and to check boxes and write statements in response to questions about medical treatments that one may or may not want under certain circumstances.
Think of it as a living will or as a conversation piece you can use to collect all your end-of-life wishes in a single place. Five Wishes is a legal document in all states but eight. Alabama, Indiana, Kansas, New Hampshire, Ohio, Oregon, Texas and Utah all require their own official documentation.
Take the following steps to use Five Wishes: Review the document, possibly with your family as well. Fill it out in ink. Follow directions for signing it. Discuss it with your health care agent and doctor and give each of them a copy. Make sure a copy of your Five Wishes is placed in your medical file by your doctor.
I want to be pain free. I want you to help me die gently and naturally. I want to spend my last days in the hospital. I want to die at home. I want hospice care. I want you to take all necessary steps to keep me alive (including with a breathing machine, artificial feeding and dialysis). Other, please explain:
