After years, I have decided to retire and close my practice. (If you are closing your practice for any other reason, this can be stated here.) The last day that I will see patients will be (insert date). I will continue to provide medical care for you prior to this closing date.
Here are a few commonly used options for signing off a letter to a doctor: Sincerely, Best regards, Kind regards, Respectfully, Yours truly,
What could happen if I dont produce a fit note to my employer? If your employer is not satisfied that you are ill, and no evidence of sickness is provided (despite requesting the same), it can withhold your statutory or contractual sick pay.
It is best to send messages that are brief, straightforward, and focus on only one medical problem or concern.
When following this convention, Yours sincerely is the most common option. It is used to end a letter when you know the last name of the person you are writing to. If we start the letter with Dear Dr Menzes or Dear Ms Brooks, then we end the letter with Yours sincerely.
A doctors note should include the date you saw the doctor, that you had a valid reason for missing work, any limitations they recommend and if a period of absence from work is needed.
2. Add a closing Best. Best wishes. Kind regards. Many thanks. Stay well. Thank you. Thanks. Warmly.
Depending on the situation, you can use formal ways to conclude a business letter: Faithfully. Sincerely. Best regards. Thank you. With appreciation. With gratitude. With sincere thanks. Sincerely yours.
