The Lachman test is a passive accessory movement test of the knee performed to identify the integrity of the anterior cruciate ligament (ACL). The test is designed to assess single and sagittal plane instability.
The anterior Lachman test, anterior drawer test and the pivot shift test, which are summarised in Table 1, are the most commonly known physical tests used to assess the integrity of the ACL (Benjaminse 2006; Leblanc 2015; Malanga 2003; Scholten 2003; Solomon 2001).
Data for the physical examination are summarized in Table 1. A positive Lachman test or pivot test is strong evidence of an existing anterior cruciate ligament (ACL) tear, and a negative Lachman test is fairly good evidence against that injury.
The mean thickness in the middle third of the ACL was 4.5 mm in the sagittal plane (range: 3.17.2 mm) and 4.3 mm in the frontal plane (range: 2.96.2 mm). The mean size of the lateral femoral condyle was 62.2 mm (range: 48.174.7 mm), while the mean size of the medial femoral condyle was 55.7 mm (range: 43.267.4 mm).
The anterior cruciate ligament (ACL) is one of the key ligaments that help stabilize the knee joint. The ACL connects the thighbone (femur) to the shinbone (tibia).
Anatomy. The ACL and PCL are two major ligaments that crisscross within the joint, allowing the knee to flex and extend without sliding back and forth. The ACL prevents the tibia from sliding forward along the femur, while the PCL prevents the tibia and femur from sliding backwards.
The anterior cruciate ligament runs diagonally in the middle of the knee. It prevents the tibia from sliding out in front of the femur and provides rotational stability to the knee. The PCL keeps the shinbone from moving backward too far. It is stronger than the ACL and is injured far less often. Normal knee anatomy.
ACL Classification Injuries are graded based on severity: Grade 1, Grade 2, and Grade 3. Grade 1 injuries are the least severe, and Grade 3 injuries are the most severe. All three grades are varying levels of a sprain of the ACL. Sprains refer to ligament injuries, whereas strains refer to muscle injuries.
Katz and Fingeroth reported that the knee anterior draw test has a diagnostic accuracy of acute ACL ruptures (within 2 weeks of examination) of: 22.2% sensitivity and 95% specificity.
The Lachman test is the most accurate test for detecting an ACL tear. Magnetic resonance imaging is the primary study used to diagnose ACL injury in the United States. It can also identify concomitant meniscal injury, collateral ligament tear, and bone contusions.
