Inform your employer about your name change so that they can update their payroll and HR records ingly. You may need to provide them with a copy of your court order or marriage certificate. Remember, this is not an exhaustive list, and other important forms and documents may need updating as well.
Name and current home address:The employee should list their full legal name and a current address where you can send their W-2. Social security number:An employee without a social security number can submit a W-4 if you direct them to fill out Form SS-5 to register for a social security card.
Notify HR/Payroll Department This commonly includes the new Social Security card, document of name change (i.e. marriage certificate), and tax forms (i.e. Form W-4, Form I-9). This process also includes confirming the employees banking information is updated and that they receive accurate paychecks.
The Internal Revenue Service (IRS) requires that the name on the Social Security card match the name on W-4 and W-2 forms. Update the employees benefits paperwork. If the change is related to marriage or other qualifying event, the employee may also want to change elections or add or remove a spouse or dependent(s).
If an employee has not updated his or her Social Security card, the old name should remain in the employers payroll system until the name is updated. Employees will also need to complete a new W-4.
If your employee is moving or changing their name, you need to update the employees information sooner rather than later, to ensure Forms W-2 and W-4 have the correct information associated with it. Additionally, employers should update section 3 of the employees I-9 form with their new name.
You can do so by asking to see their new Social Security Card that shows the name change (it typically takes 14 days for them to receive their new card). If it does reflect a name change, instruct the employee to submit a new Form W-4 with their new legal name, and then update payroll ingly.
Typically, employers have an HR name change form that an employee must complete, as well as submit the proof of identity and name change forms. This commonly includes the new Social Security card, document of name change (i.e. marriage certificate), and tax forms (i.e. Form W-4, Form I-9).
