hey guys so today were going to be doing a patient intake video you guys have been requesting this video so here we go so um a couple different things I want to show you before I get started I have a paper chart here and the paper progress note so you most likely may be working somewhere where they are using electronic health records but just in case I do want to show you how I would look if youre using paper so we have our patients name and date of birth most progress notes will also have medical record number Chief complaint we know is what the patient is coming in for we got blood pressure post respiration temperature spo2 weight and height okay now all of this information is what you want to see me go over to babe Im going to ask her what shes coming in for her medications if shes taking any any allergies now anytime you have a female patient you always want to ask the first day of the last period okay medical history any illnesses or surgeries family history now if your pati
