Rules for DNR Orders Follow these general rules to ensure the validity of your DNR order: They cannot be verbally issueda DNR must be documented on a standard form and signed appropriately. DNR protocols typically require the signing of an out-of-hospital DNR order by both the clinician and patient (or the patients surrogate) and the use of a special identifier (eg, a bracelet or brightly colored form) that is worn by or kept near the patient.
In most cases, a child will have both a DNR on file at the hospital, as well as a second pre-hospital form that he/she will keep on hand at all times. The problem with a standard DNR is that it often doesnt work well for children who have complex medical needs.
Regardless of the format or the venue, DNR orders almost always follow the same general rules to be valid: DNR orders must be written by medical professionals rather than verbalized. Written DNR orders must include the patients name. DNR orders must be dated. The DNR order must be signed by the healthcare provider.
The doctor can fill out the form for the DNR order. The doctor writes the DNR order in your medical record if you are in the hospital. Your doctor can tell you how to get a wallet card, bracelet, or other DNR documents to have at home or in non-hospital settings.
A DNR is a request not to have CPR if your heart stops or if you stop breathing. You can use an advance directive form or tell your doctor that you dont want to be resuscitated. Your doctor will put the DNR order in your medical chart. Doctors and hospitals in all states accept DNR orders.
In summary, the minimum requirement in order for a DNR form to be valid is the valid signature and date of the patient, or of their legally recognized healthcare agent in the event that they are incapacitated or otherwise unable to express their wishes.
