An individuals full name is their whole, legal name. It includes their full first name (forename), middle name(s) or initial(s) if any, and their full last name (surname). Usually, a persons full name is the name stated on their passport or drivers licence.
You must provide the FULL NAME (including any middle names), address and date of birth of each attorney.
It should contain your first and last name, so your middle name is not essential. Your signature should also be clear and readable, resembling your signature on your other important legal documents.
You could print out a photocopy first to fill in before you do the real one. Just make sure you take a photocopy of each page before you send them off and dont forget the cheques for payment. I also suggest sending them by recorded delivery for extra peace of mind.
With an unregistered LPA any mistakes must be crossed through and written again nearby. The corrections must be initialled by the person completing that particular section of the form and their witness if appropriate.
In any event, if an individual wants to get rid of her middle name, legally and forever, its entirely possible in all states.
It is not possible to amend an EPA or LPA once signed. Any changes would require a new document. However a change of address does not require an updated document. We would recommend that you send a note, signed by you, with the new details, to whoever has prepared your EPA or LPA normally this is a law firm.
If you or an attorney change name or address Do not send any original documents. You must tell OPG if you or your attorney changes address, but you do not need to send any supporting documents. Do not make changes to your lasting power of attorney (LPA) document itself, as it might become invalid.
Full legal name means an individuals first name(s), middle name(s), and last name(s) or surname without the use of initials or nicknames unless otherwise acceptable in Section 5.0 of this document.
It costs 82 to register an LPA unless you get a reduction or exemption. You can cancel your LPA if you no longer need it or want to make a new one.
