so lets make a quick google form im going to go to drive.google.com im going to click on new more google forms this is my quick form and i like to do first name and look how cool this is the ai reads and says hey you know what i think you dont want multiple choice on this you want short answer and then im coming over to the floating toolbar and i click the plus icon to add a question so im going to do last name now youll notice its already asking for email address and thats because up here in the settings i have under general it says collect email addresses response receipt sends them their response so i dont want that im going to turn that off im going to go ahead and click save the other thing you can do is you can go to the three dots and go down to preferences and you can default to collect email addresses so youll notice that when i made this new google form that already was asking for email addresses now how do i share this form thats super important because right n

California Labor Code Section 432 requires an employer to produce all documents an employee signed. Although there is no specified time requirement nor penalty specified, it is reasonable to expect an employer to produce signed documents within 30 days of receiving the request.
A write up template is basically divided into four parts namely, the heading, disciplinary levels, subject and prior notification sections. There is a section which can be changed depending on the issues to be addressed and on the other hand there is the Main Template Document, which does not change.
Written request required: Yes. If employee makes an oral request, the employer must supply a form to make a written request. Conditions for viewing records: Employee may view personnel file at reasonable times, during break or nonwork hours.

In California, there are no specific laws that require employers to issue a formal employee write-up. California is an at-will employment state, meaning employers are generally free to terminate an employee at any time, for any reason, or for no reason at all, as long as the reason is not illegal or discriminatory.
The employee receives a copy of the Written Warning with the facts, goals, steps for success, and the consequences of failing to correct the work issues.
Make sure each employee write-up form includes: The employees name or ID Number. The employees position. A specific recounting of the offense committed. The type of warning or discipline enacted. A plan for improvement. Any and all managers or decision-makers who should be aware of the situation.
If there was a written document placed into your employee file or elsewhere there is a process in the Labor Code where you can request copies of your file, and another that requires the employer to provide you with a copy of all documents you have signed.
