Hi everyone, Kevin here. Today I want to show you how you can create your very own fillable form using Microsoft Word. First off, what is a fillable form? You can have people go in and fill out a form, but they wont be able to edit the questions or the structure of the form. You can use things like check boxes, text fields, drop down lists, date pickers, to build your form. Once someones finished completing your form, they can e-mail it back, they could print it out, or you could even connect it to a database. Connecting to a database is outside of the scope of todays tutorial. Its kind of like an docHub PDF form, but its Microsofts version of it. Ive included sample files today if you want to follow along, otherwise lets jump on the PC and lets get started. The write-up form should contain: The employees name, position, official job title, and ID number if applicable. Whether this is the first written warning (or other number) What the employee did that warranted a write-up, in simple terms (for example, the employee had excessive absences)
Here are a few steps on how to write up an employee: Begin with the basic information. Generally, the top of the document includes the employees name, job title, department and ID number. Be specific when describing the incident. Use witness statements. Create a detailed improvement plan. Sign the document.
The write-up should focus on the specific objectives of the project, the methodology used, and the major findings. Regarding title, instead of giving a general, topic-like title, be specific and emphasize the explicit nature of the work.
While it is best practice to provide the writing to the employee, the law does not require an employer to provide the employee with the write up at the time it is given to the employee.
Example of How to Write up an Employee Description of the Issue: On [date], at approximately [time], I observed that you arrived to work 20 minutes late. Corrective Action: This written warning serves as a final notice regarding your tardiness. Improvement Plan: Consequences of Not Improving:
