A reasonable fee for providing copies of medical records in electronic format is a charge of no more than: $25 for 500 pages or less and $50 for more than 500 pages. Also, a reasonable fee of up to $15 may be charged for executing an affidavit, if requested.
A provider must disclose a medical record within a reasonable period of time after receiving your written request. Under state law, you may be charged no more than 76 cents per page for having your records copied, plus actual postage and handling costs. (These limitations do not apply to x-rays.)
A covered entity may charge individuals a flat fee for all requests for electronic copies of PHI maintained electronically, provided the fee does not exceed $6.50, inclusive of all labor, supplies, and any applicable postage.
The Privacy Rule permits a covered entity to impose a reasonable, cost-based fee to provide the individual (or the individuals personal representative) with a copy of the individuals PHI, or to direct the copy to a designated third party. The fee may include only the cost of certain labor, supplies, and postage: 1.
The law recognizes that health care providers will incur some expense in providing access and permits recovery of these costs. If copies are requested, there may be a reasonable cost-based charge for fees associated with the copying of the medical records including labor and copy fees.

4-304. -- Page 7 (1) Except as otherwise provided in this subtitle, a health care provider shall comply within a reasonable time after a person in interest requests in writing: (i) To receive a copy of a medical record; or (ii) To see and copy the medical record.
